Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2, which allows for multiple and varied meta-epidemiological research studies to evaluate the effect of treatment on trials that have different levels of pragmatism and other design features.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the usage of the term "pragmatic" is not consistent and its definition and assessment requires further clarification. Pragmatic trials should be designed to inform policy and clinical practice decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic trial should also strive to be as close to the real-world clinical environment as possible, including in its participation of participants, setting up and design, the delivery and execution of the intervention, and the determination and analysis of the outcomes, and primary analyses. This is a major distinction between explanatory trials as described by Schwartz & Lellouch1 that are designed to test the hypothesis in a more thorough way.
Trials that are truly pragmatic must avoid attempting to blind participants or healthcare professionals, as this may result in distortions in estimates of treatment effects. Practical trials should also aim to enroll patients from a wide range of health care settings so that their results can be applied to the real world.
Furthermore, pragmatic trials should focus on outcomes that are important for patients, such as quality of life or functional recovery. They are clinical trials that are randomized which compare real-world treatment options rather than experimental treatments under development, they have patients that more closely mirror the ones who are treated in routine care, they employ comparators that are used in routine practice (e.g., existing drugs) and depend on participants' self-reports of outcomes. This approach can help overcome the limitations of observational studies which include the biases associated with reliance on volunteers and limited availability and the variability of coding in national registries.
Pragmatic trials also have advantages, including the ability to draw on existing data sources, and a greater probability of detecting meaningful distinctions from traditional trials. However, pragmatic trials may still have limitations that undermine their credibility and generalizability. For example, participation rates in some trials could be lower than anticipated due to the healthy-volunteer effect as well as incentives to pay or compete for participants from other research studies (e.g. industry trials).
